1.1 If the website offers an extra secure environment, you must first register in order to use all the functionalities of the website. After your registration has been completed and your account has been approved, if necessary, by GP A.S. Eijtinger, you can log into your account directly and use the services offered in full.
1.2 If you have an account, you must protect access to your account from unauthorised persons by means of the user name and password. In particular, you must keep the password strictly confidential. GP A.S. Eijtinger may assume that everything that happens from your account after logging in with your user name and password is done under your direction and supervision. You are therefore liable for all such actions, unless you have reported to Huisarts A.S. Eijtinger that another person knows your password.
1.3 If necessary for the service you have requested, GP A.S. Eijtinger may request additional identifying information from you before the requested service is provided.
1.4 GP A.S. Eijtinger reserves the right to refuse registrations without giving reasons.
1.5 The medical information on the website is for general information purposes only and can never be considered a substitute for a consultation with a specialist. For personal advice, always contact GP A.S. Eijtinger.
1.6 Your personal data may be processed via the website. Please refer to the privacy statement of GP A.S. Eijtinger for more information.
2.1 It is prohibited to use the website for actions and/or conduct that violate Dutch or other applicable laws and regulations or public order and morality.
2.2 If GP A.S. Eijtinger observes that you are in breach of the above conditions, or receives a complaint about this, it will warn you. If this does not lead to an acceptable solution, Huisarts A.S. Eijtinger may intervene itself to end the breach. In urgent or serious cases, GP A.S. Eijtinger may intervene without warning.
2.3 If, in Huisarts A.S. Eijtinger's opinion, there is a nuisance, damage or other danger to the functioning of the computer systems or the network of Huisarts A.S. Eijtinger or third parties and/or to the provision of services via the Internet, in particular due to excessive sending of e-mail or other data, leaks of personal data or activities of viruses, Trojans and similar software, Huisarts A.S. Eijtinger is entitled to take all measures that it reasonably considers necessary to avert or prevent this danger.
2.4 Huisarts A.S. Eijtinger is at all times entitled to report any criminal offences observed. Furthermore, Huisarts A.S. Eijtinger is entitled to hand over your name, address, IP address and other identifying data to a third party who complains that you are infringing its rights or these Terms of Use, provided that the accuracy of that complaint is sufficiently plausible in all reasonableness, there is no other way to obtain this data and the third party has a clear interest in handing over the data.
2.5 GP A.S. Eijtinger reserves the right to recover from you any damages resulting from violations of these terms of use.
3.1 GP A.S. Eijtinger actively maintains the website. If maintenance is expected to limit availability, Huisarts A.S. Eijtinger will carry it out when the number of visitors is relatively low. Maintenance in connection with emergencies may take place at any time.
3.2 GP A.S. Eijtinger may modify the website and the functionalities on the website from time to time.
4.1 The website, the associated software as well as all information and images on the website are the intellectual property of Huisarts A.S. Eijtinger or its licensors. These may not be copied or used in any way without the separate written consent of Huisarts A.S. Eijtinger, except in the cases in which this is permitted by law.
5.1 Once you create an account, you enter into an agreement with GP A.S. Eijtinger. This agreement is valid indefinitely and gives you the right to use the services on the website subject to these terms of use.
5.2 You may terminate the agreement at any time with immediate effect by notifying GP A.S. Eijtinger by e-mail.
5.3 GP A.S. Eijtinger is entitled to terminate the agreement if you have not used your account for eighteen months. In this case, it will first send a reminder e-mail to the e-mail address linked to your account.
6.1 GP A.S. Eijtinger may amend these terms and conditions at any time.
6.2 GP A.S. Eijtinger will announce changes or additions to these Terms of Use at least fourteen days before they take effect via the website so that you can take note of them. Amendments of minor importance will take effect without announcement.
6.3 If you do not wish to accept an amendment or supplement, you can cancel the agreement until the date of entry into force. Use of the website after the date of entry into force shall constitute acceptance of the amended or supplemented terms and conditions.
7.1 These terms of use are governed by Dutch law.
7.2 To the extent not otherwise prescribed by the rules of mandatory law, all disputes related to the website will be submitted to the competent Dutch court for the district in which GP A.S. Eijtinger is established.
7.3 If you request an eConsult or repeat prescription via Huisarts A.S. Eijtinger, this is subject to the 2007 Guideline on online doctor-patient contact. You can view this guideline on request, or download it from the KNMG website.
7.4 If any provision in these Terms of Use is found to be invalid, this shall not affect the validity of the entire Terms of Use. In this case, the parties shall determine (a) new provision(s) to replace it(them), which will give shape to the intention of the original provision as much as legally possible.
7.5 GP A.S. Eijtinger is entitled to transfer its rights and obligations under the agreement to a third party who takes over the website or the relevant business activity from it.
7.6 Deviations from these terms of use are only binding if they have been accepted in writing by GP A.S. Eijtinger.
7.7 Questions arising after reading these terms of use can be asked via the practice.
Practices are very busy. The phone is red hot at many times and the consulting hours are overcrowded. We are aware that you sometimes have to wait a long time on the phone and also sometimes have to wait before you can come to the surgery. We prefer not to do this either.
The demand for care is currently very high. We do our best to help everyone as much as possible.
